  • I am an Out-of-Network (OON) provider with all commercial insurance companies including Medicare Advantage plans (Part C Medicare Plans). I am contracted with Original *Medicare (Part B Medicare plans) as a Non-Participating Medicare Provider. This means that patients who schedule with me, pay for their visit at the time of their appointment.

    *If you have an a Part B Medicare plan please reference the Medicare Physical Therapy Patients Tab Below, for additional information about treatment rates.

    Treatment Rates:

    • Initial Evaluation (60 Minutes): $250

    • Follow-up Visit (60 Minutes): $200

    After over 5 years of practice in the insurance based medical system, I choose to become an Out-of-Network Provider with commercial insurance companies, to prioritize quality patient care. Being an Out-of-Network provider allows me to offer individualized treatment that is not dictated by your insurance company. It allows me to offer more holistic and trauma informed care, that isn’t rushed in a short appointment. Instead of the common Physical Therapy appointment model in which appointments are 30-40 minutes, or even double booked with another patient, appointments with me are actually 60 minutes. I work with one patient at a time for the entire appointment.

    Many insurance plans have Out-of-Network benefits. Depending on your insurance plan, seeing me as an Out-of-Network Physical Therapist may actually save you money.

    I can provide you a superbill upon request, to submit to your insurance company for reimbursement. Please note, reimbursement amount depends on your insurance plan and its OON benefits.

    I understand that being an OON provider is a barrier to care for some patients. Pelvicrehab.com is a free online directory you can use to search for another pelvic floor therapist who may be in-network.

  • Unlike other health insurance companies, The Center for Medicare Services (CMS) does not allow Physical Therapists to be “In-Network” or “Out-of-Network” with Part B Medicare Plans. Instead, CMS gives Physical Therapists three options:

    1. Apply to be a Participating Medicare Provider

    2. Apply to be a Non-Participating Medicare Provider

    3. Have no relationship with medicare. Physical Therapists who have no relationship with Medicare are legally restricted in treating Medicare patients for services that are normally covered by Medicare.

    I have chosen to be a Non-Participating Medicare provider, because it is important to me to keep serving patients with Part B Medicare Plans.

    If you have a Medicare Advantage Plan (Part C), or a supplemental Medicare Plan, insurance coverage works a little differently. If you have a Part C plan/a Supplemental Plan, I will likely be considered an “Out-of-Network” Provider instead of a “Non-Participating” Medicare Provider.

    Regardless of what type of Medicare Plan you have (Part B with or without a supplemental plan, or a Medicare Advantage Part C plan) if you schedule with me you will pay for the visit at your time of service- but at a discounted rate.

    Estimated Medicare Patient Treatment Rates (discounted based on the CMS Fee Schedule) are approximately:

    • Initial Evaluation (60 Minutes): $200

    • Follow-up Visits (60 Minutes) $150-180

    Following your visit, I will submit your Physical Therapy claim to Medicare or your Medicare Advantage plan on your behalf, so that your insurance plan may reimburse you according to your specific benefits.

    Please note, we cannot guarantee what Medicare or your Medicare Advantage plan will reimburse you. because reimbursement rates depend on your individual insurance plan benefits. If you would like an estimate on what your insurance plan will reimburse you, we recommend you contact your insurance company directly (start by calling the customer service number located on the back of your insurance card).

    • Initial Evaluation (60 minutes): $250

    • Follow-up visit (60 minutes): $200

  • Yes, I provide Physical Therapy superbills for Non-Medicare patients upon request, that you may submit to your insurance for possible reimbursement of services, if you have out-of-network benefits.

    I don’t provide superbills for Medicare patients, because The Center for Medicare Services designates that Non-Participating Providers submit Physical Therapy claims to Medicare on the patients behalf.

    • 60 minutes Reiki Session: $125

    Shorter Reiki Sessions may be available upon request. Contact info@cedarphysicaltherapy.com with requests for shorter visits.

  • Unfortunately most insurance companies do not recognize Reiki as a covered treatment modality. As a result, I am unable to provide a superbill for Reiki services or couples sexual wellness sessions at this time.

  • Please provide 48 hours notice prior to cancelling an appointment. Appointments cancelled with less than 48 hours notice will be charged the full appointment fee.

    I understand emergencies and illnesses happen; please reach out to me via email or phone if you need to cancel due to an emergency or illness.

  • I work with people experiencing the following symptoms:

    • Urge Urinary Incontinence

  • Yes! I strive to help you better understand the normal hormonal, cardiovascular, respiratory, and musculoskeletal changes that occur in your body throughout your pregnancy. I will work with you to help you create a prenatal exercise routine that is right for you. I consider it an honor to help patients prepare for labor and birth as well as supporting your post-partum health. I frequently collaborate with doulas, midwives, OBGYNs and other members on a birth team.

  • Yes! Creating a safe and private environment for you to receive pelvic health services is important to me.

  • Your comfort and safety are my top priority. I understand that unfortunately many people with vaginas have had negative health care experiences when it comes to internal vaginal exams. I recognize that many people with vaginas are survivors of emotional, physical, and sexual abuse.

    I believe in always asking for consent, and I believe consent is not a “one time thing.” Rather, consent is an ongoing practice. You are always welcome to bring a friend, family member, or partner to your pelvic health PT appointments, if this is something a patient feels will support you in feeling safer and more comfortable. I otherwise practice informed consent throughout all of our appointments.

    You can expect me to explain what each treatment technique entails prior to beginning the exam or treatment technique.

    I will check in with you throughout the treatment to see if you would like to pause, take a break, or change the treatment plan at any time.

  • Intravaginal pelvic floor muscle examinations can provide a lot of helpful information regarding the most beneficial treatment plan for you. For example, they can help you and me determine if your pelvic floor muscles are weak, tight, underactive, or overactive. Treatment will vary slightly based on what you and I discover together about your pelvic girdle and pelvic floor muscles during the pelvic floor muscle exam.

    Intravaginal pelvic floor muscle exams can also help you and me determine if there are signs of pelvic organ prolapse, skin dehydration, or skin irritation- which are all important to consider when creating the best individualized treatment plan for you!

  • Yes! It is quite common for people who are experiencing pelvic health symptoms to also experience hip pain, low back pain, knee pain or pain in other areas. As such, I will complete a comprehensive exam that assesses the entirety of your health, not just your pelvic floor! Even if you are not experiencing hip pain, low back pain or knee pain, examination of these regions may be beneficial in treating your pelvic health symptoms. I will explain options for my recommended treatment plan during each appointment, and you will have an opportunity to provide informed consent for all treatment plans.

  • Absolutely not! There is so much we can accomplish together if you would prefer not to do an intravaginal pelvic floor muscle exam or an anorectal pelvic floor muscle exam.

    I encourage my patients to choose the type of pelvic floor muscle exam that feels most safe and comfortable for them. At your initial evaluation I will explain your options for a comprehensive pelvic floor muscle exam (verbal, over clothes or intravaginal/anorectal pelvic floor muscle exam) and you will have the opportunity to choose which type of exam is best for you.

  • Yes! I provide several lubricant options during intravaginal pelvic floor muscle exams including:

    • Good Clean Love

    • Uber Lube

    • Organic Cocount oil

    You are welcome to bring your preferred lubricant to sessions as well.

  • Yes, I treat all genders, ages 10+.

  • Yes, I offer gender affirming care and have experience treating patients across the gender spectrum.

    As a Cisgendered woman, I have not had to seek out gender affirming care for myself, however, being a Black Biracial and Bisexual human, I do know what it’s like to not feel seen, safe and advocated for in a health care environment.

    it’s very important to me to create an environment that feels safe and healing for marginalized individuals.
